Results of the Primary Noncemented Porocoated Hip Replacement Arthroplasty




Abstract
Problems in cemented total hip arthroplasty(THA) such as increased incidence of mechanical loosening, extensive bone loss sometimes associated with cement fragments, and difficulty of revision have made cementless THA to replace much of cemented
one.
From 1986 to 1990, primary noncemented porocoated THA's were performed in 441 hips of 374 patients at the department of orthopedic surgery of Seoul National University Hospital. The results of the procedures were analysed for 238 hips of 188
patients
which were followed up than 2 years.
The causes were avascular necrosis of the femoral head in 150(63%) hips, infection sequelaes in 36(18%) hips, fused hip in 11(5%) hips, fracture of the hip in 10(5%) hips, primary degenerative arthritis in 8(3%) hips and others in 23(11%) hips.
Average
Harris hip score was improved from 64 preoperatively to 90 postoperatively. Average hip pain was in 29(14%) hips, and thigh pain was in 11(5%) hips. Early complications were crack through the femoral neck in 13(6%) hips, nerve palsy in 8(3%)
hips,
superficial infection in 4(2%) hips, deep infection in 2(1%) hips and dislocation in 4(2%) hips. Late complications were ectopic ossification in 24(10%) hips and nonunion of the greater trochanter in 6(2%) hips. Osteolysis in 25(11%) hips and
loosening
in 7(3%) hips.
